Is there a different repair form/process for warranty claims?

Regardless of warranty status, all repairs will use the same repair form found here: Repair Page

The repair form will automatically show pricing outside of warranty. However, until a warranty determination is made, the only fee you are responsible for is the processing fee. Please be sure to include your purchase month and year on the summary page of the repair form to indicate your glasses are within the two-year warranty period. Please also include a note requesting “Please evaluate for warranty” in the notes section of the summary page.

If repairs are found to be outside of warranty, we will contact you to discuss the next steps before any additional fees are incurred. To only include payment for the processing fee, you can leave the 'charge approval' box unchecked or click the 'pay by check' box at the end of the form.
