Upon completion and submission of the online repair form, you will be sent a confirmation email that contains your repair form and the shipping details. This email can sometimes end up in your spam or junk mail folder as it comes from a "Do Not Reply" address. You can either print off the repair form using the link provided within the email or print off the email itself.
If you did not receive your completed repair form in your email, you may contact us via phone or email. We would be happy to assist you to ensure your repair form was submitted and provide the information to include in the box with your sunglasses.
